Mallorca opened the scoring after just 21 minutes played through Daniel Rodriguez and took their lead into the break.
However, Bilbao scored just five minutes after the return from recess to tie the score.
There were opportunities for Nico Williams to seal the win for Ernesto Valverde's side before the shootouts but the Spain international fluffed his finish as the game headed into penalties.
During the decisive moments, Mallorca missed two of their four kicks while Bilbao scored all of their first four to clinch the trophy.
The triumph ends a 40-year drought without winning a major trophy for the club.
Inaki was key to Bilbao's run to the trophy, scoring two goals and registering two assists in five appearances in the cup competition.
