The National Service Authority (NSA) has extended the deadline for the submission of Monthly Evaluation Forms by National Service Personnel (NSP) to Friday, February 6, 2026.
In a press release issued by the Authority on Friday, January 30, the NSA said the extension was granted to provide additional time for personnel who have yet to complete and submit their forms to do so.
The NSA has therefore urged all National Service Personnel to take advantage of the extended period and ensure that their Monthly Evaluation Forms are duly completed and submitted within the new deadline.
"This extension has been granted to allow personnel who are yet to complete and submit their forms adequate time to do so," the statement read.
The Authority further announced the introduction of an appointment scheduling system aimed at improving the submission process at NSA District Offices.
Under the new arrangement, all NSPs will be required to book an appointment on their individual dashboards before visiting their respective district offices to submit their Monthly Evaluation Forms.
"In addition, allNSPsP are required to schedule an appointment on their individual dashboards before visiting their respective NSA District Offices to submit their Monthly Evaluation Forms."
According to the NSA, the appointment scheduling feature will be activated on Sunday, February 1, 2026, allowing personnel to select convenient dates and times for submission.

The measure, the Authority explained, is intended to reduce congestion at district offices and promote a more orderly, efficient and convenient process for service personnel nationwide.
"The appointment scheduling feature will be activated on Sunday, 1st February, 2026, for alNSPsSP to schedule appointments. This measure has been instituted to curtail congestion at the offices and ensure a more orderly, efficient and convenient submission process for all service personnel," the statement read.
The NSA reiterated that the submission of Monthly Evaluation Forms remains a mandatory requirement for the validation of active service status and the processing of service allowances.
Personnel who fail to comply with the new deadline, the Authority cautioned, risk delays in the processing of their allowances.

The Authority has therefore urged all National Service Personnel to comply fully with the directives and to rely solely on official NSA communication channels for accurate and up-to-date information.
