Asamoah Gyan was replaced in the first half as Legon Cities needed a late strike from Richmond Antwi to knock out Uncle T from the MTN FA Cup.
Gyan was replaced by Baba Mahama in the 33rd minute as the former held his thigh whilst walking off.
The Royals took the lead through Antwi minutes after Gyan's depature.
Uncle T equalised from the penalty spot right on the stroke of halftime.
The game looked set to head to penalties with both sides lacking the cutting edge.
Antwi completed his brace with a brilliant strike two minutes from full time.
Legon Cities qualify to the round of 32 stage of the competition.
In other games, Kato Freedom Fighters FC beat Bechem United 1-0.
